This policy explains how Base Agency Inc. collects, uses, shares, and protects personal information through 1099protect.com (the “Site”), our agency producer onboarding workflows, and the participant enrollment process for the WORK Insurance, Trucking and Last Mile Delivery, and Truckers Insured (Legacy) programs.
2. Information We Collect
Information you provide directly:
- Agent appointment data — name, agency name, email, phone, physical address, NPN, resident state, uploaded resident state license (PDF), E&O certificate, voided check, W-9, electronic signature.
- Driver / participant enrollment data — name, contact info, license number, employment classification documents, application forms.
- Contact form submissions — name, email, message, attachments.
- Upload Center submissions — completed application packages including third-party personal information about the contractors being enrolled.
Information collected automatically:
- Cookies, browser type, IP address, device data, pages visited, referral source, session timing.
- Google Analytics 4 (measurement ID G-NEZ3RD74MD) for site usage analytics.
Information from third parties:
- Verification data from licensing authorities (NPN status, license validity).
- Agency partner submissions on behalf of their clients.
3. How We Use Information
We use collected information to process agency producer appointments; bind, service, and renew occupational accident coverage; process payments via our billing provider; verify licensing and compliance; communicate program updates; analyze site usage to improve operations; and meet legal and regulatory requirements.
4. How We Share Information
We share personal information only with parties who need it to deliver services or who are legally entitled to it:
- Carrier and program partners — to underwrite, bind, and service coverage
- Billing processor — ePayPolicy, for premium collection
- E-signature platform — WeSignature, for execution of producer agreements and enrollment documents
- Workflow automation — Make.com, Airtable, and Google Workspace, for internal processing of applications and documents
- Analytics — Google Analytics 4, in aggregated and de-identified form
- Regulatory authorities — when required by law (DOL audits, state insurance department requests, court orders)
We do not sell personal information to advertisers or data brokers.
5. Cookies and Tracking
The Site uses cookies to maintain sessions, remember your preferences, and measure traffic. You can disable cookies in your browser settings, but some site functionality may be limited as a result.
6. Data Security
We use commercially reasonable technical and administrative safeguards to protect personal information, including encrypted transmission (HTTPS/SSL) and access controls. No method of transmission or storage is 100% secure. Sensitive uploads (license PDFs, voided checks, W-9s, completed applications) are processed through encrypted channels and stored in access-restricted systems.
7. Data Retention
We retain personal information for as long as needed to administer your account, comply with our legal obligations, and resolve disputes. Producer files, applications, and policy records are typically retained for the duration of the program relationship plus the period required by state insurance law (typically 5 to 10 years).
8. HIPAA Notice
Base Agency Inc. is not a “covered entity” under the Health Insurance Portability and Accountability Act (HIPAA). However, when our processes handle protected health information in the course of insuring healthcare-staffing contractors, we treat that information with the same encryption, access-control, and retention safeguards applied to all sensitive personal data.
9. Your Rights
Depending on where you live, you may have rights to access the personal information we hold about you, correct inaccurate information, request deletion of certain information, opt out of certain disclosures, and receive a copy of your information in portable format. California residents have rights under the CCPA. EU and UK residents have rights under GDPR. To exercise any of these rights, contact us using the information below.
10. Children’s Privacy
The Site and our services are not directed to anyone under 18. We do not knowingly collect personal information from children.
11. Changes to This Policy
We may update this policy from time to time. Material changes will be reflected by updating the “Effective date” at the top. Continued use of the Site after the update constitutes acceptance of the revised policy.
